The specifics of this methodology have been reported previously.11,14–17 For this analysis normal the PV was defined pre hoc as measured volumes of +10% or less of normal expected volume, mild to moderate expansion as greater than +10% to less than +25%, and large volume expansion as +25% or more above normal volume based on previously reported data.14,18,19
