Storiform collagenoma (sclerotic fibroma) of the oral mucosa

Abstract

Storiform collagenoma (also called sclerotic fibroma) is uncommon, occurs as a cutaneous fibrous neoplasm, and is particularly rare in mucosal tissue in the head and neck. We describe an unexpected diagnosis in the oral cavity. Histopathological examination showed a proliferation of fibrous tissue, which was well circumscribed but unencapsulated, with thick laminated bundles of eosinophilic collagen that exhibited a storiform or “whorled” pattern. First described by Weary et al in 1972, storiform collagenoma is a marker for Cowden’s disease or PTEN hamartoma tumour syndrome. Identification of other synchronous lesions should prompt chromosomal analysis for a mutation in the PTEN gene on chromosome 10q23.

Introduction

Storiform collagenoma (also called sclerotic fibroma) is uncommon. It occurs as a cutaneous fibrous neoplasm, but appearances at mucosal sites in the head and neck are particularly rare.1, 2, 3 The diagnosis is important, however, as a marker of PTEN hamartoma tumour syndrome or Cowden’s disease, but only when present at multiple sites.4

Case report

A 50-year-old woman presented with 12 months’ history of a slow-growing mass on the hard palate. Clinical examination showed a sessile, firm mass that measured roughly 1 cm in diameter and had normal overlying mucosa. A panoramic radiograph showed no evidence of any abnormality and the presumed diagnosis was a tumour of the minor salivary gland. Discussion

Storiform collagenoma or sclerotic fibroma is a benign tumour of the soft tissue, which usually presents as a slowly-enlarging, well-circumscribed mass at cutaneous sites of the face and limbs, although oral mucosal presentations have been published.1 Classically, the histopathological description is of concentrically lamellar patterns or a “plywood-like” arrangement of collagen bundles.5 There are usually positive staining for CD34 and vimentin on immunohistochemical analysis.2
