Alternative endoscopic treatment of Zenker's diverticulum: a case series (with video)

Methods

We performed a prospective study in a single Italian center, the New S. Agostino-Estense Civil Hospital, Baggiovara, Modena, Italy. The study was approved by the local institutional review board, and each patient gave signed informed consent for the procedure.

Results

During the study period, 21 procedures in 19 patients were performed. The median age of the patients was 74 years (range, 46-84 years), with a male-to-female ratio of 13:6. The median size of the diverticulum was 42 mm (range, 30-55 mm). The mean procedure time was 22 ± 5 minutes.

The median follow-up time was 27 months (range, 3-48 months). During follow-up, dysphagia relapsed in the first 2 cases (6 and 8 months from the procedure, respectively). The size of the recurrent diverticula was 15

Discussion

This is the first report of use of the IT-Knife 2 (originally developed for endoscopic submucosal dissection) to perform Zenker's diverticulectomy. The choice of this device was based on our experience in endoscopic submucosal dissection. In our experience, for Zenker's diverticulectomy the IT-Knife 2 allows better control and precise cut compared with other devices, allowing a more stable position by putting the insulated rounded tip on the septum of the diverticulum and cutting it toward a
