The patient presented with extensive necrotic, leathery, violaceous lesions on both calves that resembled eschar. Also noted were adjacent discrete focal violaceous lesions in evolution.

Involvement of blood vessels of this caliber is most specific for calciphylaxis (hematoxylin and eosin, × 20).
